ENTEROSCOPÍA ASISTIDA POR BALÓN
Es un examen que tiene como objetivo el estudio y diagnóstico de las enfermedades del intestino delgado. Se lleva a cabo con la ayuda de endoscopio más largo llamado enteroscopio. Para el ingreso por el intestino delgado se requiere colocar un sobretubo de silicona que tienen un balón en su extremo, lo cual permite plegar el intestino delgado.
Se puede llamar también enteroscopía a balón simple, enteroscopía a doble balón o enteroscopía asistida por dispositivos.
El procedimiento de enteroscopía se puede realizar ingresando por la boca o ingresando por el recto, según donde se estime que se ubique la enfermedad. La enteroscopía por la boca nos permite evaluar las dos terceras partes superiores del intestino delgado, y la enteroscopia por el recto nos permite evaluar el tercio inferior.
- Todo paciente con evidencia de una enfermedad del intestino delgado por algún método diagnóstico previo – cápsula endoscópica, tomografía o resonancia – que requiera verificar, tomar biopsias o realizar tratamientos endoscópicos.
- Toda persona que presenta alguno de los siguientes diagnósticos: Hemorragia digestiva manifiesta, diarrea crónica, dolor abdominal o pérdida de peso sin explicación luego de realizar estudios de endoscopía digestiva alta y colonoscopía, tanto para diagnóstico como para tratamiento.
- Todo paciente con anemia por deficiencia de hierro o examen de sangre oculta en heces (Thevenon) positivo, sin explicación luego de realizar estudios de endoscopía digestiva alta y colonoscopía.
- Todo paciente con sospecha o diagnóstico de Enfermedad de Crohn que requiera evaluación del intestino delgado para toma de biopsias o tratamiento específico.
- Todo paciente que tenga Enfermedad Celiaca o historia de Poliposis Familiar para seguimiento con biopsias o tratamiento.
- Todo paciente con retención de cápsula endoscópica para intentar retirarla.
Los procedimientos de enteroscopía son más complejos y largos que una endoscopía o colonoscopía, requieren siempre sedación y se utiliza insuflación con dióxido de carbono para disminuir las molestias de la distención.
- Debe advertir a su médico los medicamentos que utiliza. Algunos de ellos tienen que deber ser suspendidos hasta días antes del examen.
- Debe advertir a su médico si sospecha de estar embarazada.
- Antes de una enteroscopía debe seguir detalladamente las INSTRUCCIONES DE LIMPIEZA INTESTINAL que se le han dado. Si un paciente no tiene un intestino delgado limpio, no se puede hacer un buen examen, y en algunas oportunidades tiene que ser hasta reprogramado.
- Debe acudir a su cita de enteroscopía 30 minutos antes de su cita.
- Al llegar al examen, debe avisar a la secretaria su arribo para iniciar su preparación.
- Ingresará a la sala de recuperación donde se alistará, en ese momento deberá firmar los consentimientos informados.
- Pasará a la sala de endoscopía, donde lo recibiremos y absolveremos cualquier duda.
- Se le colocará la sedación y se iniciará el examen.
- Después del procedimiento, pasará a la sala de recuperación de donde se le dará de alta cuando este bien despierto.
- La primera comida siempre debe ser líquida. Como es un examen más largo puede haber molestias ligeras, en garganta o abdomen, se recomienda cosas frescas sin irritantes. Y la dieta blanda debe iniciarse al dia siguiente.
- La enteroscopía es un procedimiento bastante inocuo, sin embargo, como se realiza con sedación se recomienda en las siguientes 24 horas: No conducir vehículos, no realizar tareas de riesgo (Utilizando objetos cortopunzantes etc.), No realizar tareas donde requiera concentración, No tomar licor, ni consumir sedantes.
- Si en las próximas 24 horas presenta alguno de estos síntomas: dolor abdominal intenso, vómitos, fiebre o sangrado rectal, debe comunicarse con nosotros.
En manos expertas las complicaciones son muy inusuales. Sin embargo se han descrito sangrados y perforación.
