COLONOSCOPÍA TOTAL
Este examen se realiza utilizando el colonoscopio. Este es un instrumento flexible, en su punta tiene una cámara que permite visualizar la mucosa del colon. El colonoscopio se introduce a través del ano y se avanza hacia el recto, colon sigmoides, colon descendente, colon transverso, colon ascendente, ciego y finalmente la última parte del intestino delgado llamada íleon terminal.
Examinar estos órganos directamente nos permite detectar enfermedades.
A nivel del colon e ileón, se puede detectar y tratar pólipos, cáncer del recto y del colon, infecciones, divertículos, inflamaciones, úlceras, parásitos, entre otros.
- Toda persona mayor de 45 años sin síntomas (Asintomático) como estudio preventivo de cáncer de colon. (Recomendación dada por todas las Sociedades de Gastroenterología Americana y Europea). La dieta occidental – rica en comida grasosa, baja en antioxidantes – frutas y verduras – hace que 1 de cada 5 personas desarrolle adenomas colónicos (pólipos) que pueden extraerse fácilmente por colonoscopía cuando son pequeños, y si uno los deja crecer generarán cáncer.
- Se debe realizar una colonoscopía todo familiar en PRIMER GRADO de un paciente que ha sido diagnosticado de cáncer de colon y recto, o de pólipos colónicos (adenomas), la edad en que debe realizarse este examen, debe ser al menos 10 años antes que la edad en la que fue diagnosticado el familiar. Ejemplo: Si mi hermano es diagnosticado a los 40 años, mi primera colonoscopía debo programarla a los 30 años.
- Toda persona que presenta alguno de los siguientes síntomas – sin considerar la edad – Sangrado rectal, diarrea crónica con o sin sangre, estreñimiento de reciente inicio, dolor abdominal o pérdida de peso sin explicación con otros métodos.
- Todo paciente con anemia por deficiencia de hierro. O si tiene un examen de sangre oculta en heces (Thevenon) positivo.
- La colonoscopía se puede realizar con o sin sedación. La colonoscopía la realizamos con sedación para que no tenga ninguna molestia, sin embargo también se puede realizar sin sedación si el paciente lo desea.
- Debe advertir a su médico los medicamentos que utiliza. Algunos de ellos tienen que deber ser suspendidos hasta días antes del examen.
- Debe advertir a su médico si sospecha de estar embarazada.
- Antes de una Colonoscopía completa debe seguir detalladamente las INSTRUCCIONES DE LIMPIEZA COLÓNICA que se le han dado. Si un paciente no tiene un colon limpio, no se puede hacer un buen examen, y en algunas oportunidades tiene que ser hasta reprogramado.
- Debe acudir a su cita de colonoscopía 30 minutos antes de su cita.
- Al llegar al examen, debe avisar a la secretaria su arribo para iniciar su preparación.
- Ingresará a la sala de recuperación donde se alistará, en ese momento deberá firmar los consentimientos informados.
- Pasará a la sala de endoscopía, donde lo recibiremos y absolveremos cualquier duda.
- Se le colocará la sedación y se iniciará el examen.
- Después del procedimiento, pasará a la sala de recuperación de donde se le dará de alta cuando este bien despierto.
- La primera comida siempre debe ser ligera, libre de condimentos, ajíes, irritantes en general.
- La colonoscopía completa es un procedimiento bastante inocuo, sin embargo, como se realiza con sedación se recomienda: No conducir vehículos, no realizar tareas de riesgo (Utilizando objetos cortopunzantes etc.), No realizar tareas donde requiera concentración, No tomar licor, ni consumir sedantes, las siguientes 24 horas.
- Si en las próximas 24 horas presenta alguno de estos síntomas: dolor abdominal intenso, vómitos, fiebre o sangrado rectal, debe comunicarse con nosotros.
En manos expertas muy rara vez hay complicaciones. Sin embargo se han descrito los siguientes: Sangrado, perforación o necesidad de cirugía.
