CÁPSULA ENDOSCÓPICA
Este es un procedimiento que consiste en examinar el intestino delgado con una pequeña pastilla, llamada Cápsula Endoscópica, que tiene un sistema de para tomar fotos y luces integrado, y que trasmite la información a un receptor colocado en el paciente, el cual una vez terminado el examen, nos va a permitir descargar y evaluar la información.
El examen se realiza ingiriendo la cápsula, el tiempo de recorrido por el intestino delgado es variable entre persona y persona. .
La cápsula endoscópica es una pequeña computadora que se utiliza una sola vez, y se elimina de manera natural sin generar molestias, no es necesario recuperarla para evaluar el estudio. Es un procedimiento mínimamente invasivo, no requiere sedación, no produce dolor, puede realizarse con el paciente aún tomando medicación anticoagulante.
El examen tiene una duración aproximada de 8 horas, pero puede extenderse a 12 horas de ser necesario. Durante el estudio, el correcto recorrido de la cápsula endoscópica se realiza utilizando el sistema del Real Viewer, que nos permite ver donde esta la cápsula cada vez que se necesite.
El estudio lo realizamos de manera ambulatoria, no necesita hospitalizarse, pero si le pedimos que acuda a las 2 horas de ingerir la cápsula para evaluar que ya pasó el estómago. Durante las primeras 4 horas después de ingerida la cápsula, el paciente no debe ingerir sólidos, porque sino no permiten ver la superficie mucosa correctamente, según la evaluación de cada caso, le daremos las recomendaciones de como debe ingerir alimentos.
La cápsula endoscópica no reemplaza la endoscopia digestiva alta o colonoscopía, sino complementa la evaluación. La cápsula endoscópica se utiliza para examinar fundamentalmente el intestino delgado.
- Toda persona mayor de 45 años sin síntomas (Asintomático) como estudio preventivo de cáncer de colon. (Recomendación dada por todas las Sociedades de Gastroenterología Americana y Europea). La dieta occidental – rica en comida grasosa, baja en antioxidantes – frutas y verduras – hace que 1 de cada 5 personas desarrolle adenomas colónicos (pólipos) que pueden extraerse fácilmente por colonoscopía cuando son pequeños, y si uno los deja crecer generarán cáncer.
- Se debe realizar una colonoscopía todo familiar en PRIMER GRADO de un paciente que ha sido diagnosticado de cáncer de colon y recto, o de pólipos colónicos (adenomas), la edad en que debe realizarse este examen, debe ser al menos 10 años antes que la edad en la que fue diagnosticado el familiar. Ejemplo: Si mi hermano es diagnosticado a los 40 años, mi primera colonoscopía debo programarla a los 30 años.
- Toda persona que presenta alguno de los siguientes síntomas – sin considerar la edad – Sangrado rectal, diarrea crónica con o sin sangre, estreñimiento de reciente inicio, dolor abdominal o pérdida de peso sin explicación con otros métodos.
- Todo paciente con anemia por deficiencia de hierro. O si tiene un examen de sangre oculta en heces (Thevenon) positivo.
- La colonoscopía se puede realizar con o sin sedación. La colonoscopía la realizamos con sedación para que no tenga ninguna molestia, sin embargo también se puede realizar sin sedación si el paciente lo desea.
- Debe advertir a su médico los medicamentos que utiliza. Algunos de ellos tienen que deber ser suspendidos hasta días antes del examen.
- Debe advertir a su médico si sospecha de estar embarazada.
- Debe advertir a su médico si tiene dificultad para pasar pastillas o alimentos sólidos, porque en ese caso se debe programar colocar la cápsula endoscópica con ayuda de un endoscopio.
- Antes de una Cápsula endoscópica debe seguir detalladamente las INSTRUCCIONES DE LIMPIEZA que se le han dado. Si un paciente no tiene un intestino delgado limpio, no se puede hacer un buen examen, y en algunas oportunidades tiene que ser hasta reprogramado, lo cual obligará a utilizar otra cápsula endoscópica.
- Debe acudir a su cita de cápsula endoscópica 30 minutos antes de su cita.
- Al llegar al examen, debe avisar a la secretaría su arribo para iniciar su preparación.
- Ingresará a la sala de recuperación donde se alistará, en ese momento deberá firmar los consentimientos informados.
- Se le colocará el equipo de cápsula endoscópica y el receptor que deberá permanecer con usted durante todo el estudio. Si bien es un estudio ambulatorio, no debe manipular el receptor, ni los cables, porque eso podría generar pérdida de la información. Puede hacer cualquier actividad que no demande esfuerzo físico, ni retirar el equipo.
- Una vez que se coloque todo, y se verifique la correcta trasmisión de la señal de la cápsula, el paciente debe ingerirla con agua.
- Después de la ingesta, deberá volver a las 2 horas, durante ese tiempo sólo puede tomar sorbos de agua y degustar caramelos.
- Una vez se verifique el paso de la cápsula al intestino delgado, se le darán las instrucciones de alimentación y la hora en la que debe volver a dejar el equipo.
- Una vez que se regrese el receptor el paciente puede hacer su vida normal.
- Se pueden dar indicaciones específicas de acuerdo a cada caso.
- Si en las próximas 24 horas presenta alguno de estos síntomas: dolor abdominal intenso, vómitos, fiebre o distensión abdominal, debe comunicarse con nosotros.
